What to Expect at Horizen Medical

At Horizen Medical, we know many women come to us feeling frustrated, exhausted, confused, or simply unlike themselves.


Maybe you’ve noticed changes in your energy, sleep, mood, focus, weight, stress levels, cycle, or overall wellbeing. Maybe you’ve wondered if it could be perimenopause, but aren’t sure. Maybe you’ve already tried seeking answers elsewhere and left feeling dismissed or unheard. Or, perhaps, you’ve been looking for care, but just couldn’t find a clinic to call.

That’s exactly why we created Horizen Medical.


As The Perimenopause Clinic, our focus is helping women ages 35+ navigate the hormonal, physical, and emotional changes that can happen during perimenopause, menopause, and post-menopause with more support, education, and personalized care.

One thing that’s important to know about our approach is this:

We are not here to “prove” whether or not you are in perimenopause before taking your symptoms seriously.


At Horizen Medical, we believe women know their bodies best. If you tell us something feels “off,” we believe you. Our goal is not to wait until symptoms fit perfectly into a diagnostic box. Our goal is to help you better understand what may be happening and support you in feeling more like yourself again.



A More Personalized Experience


Your care at Horizen Medical starts with a 30-minute in-depth intake visit with one of our Health Coaches or Medical Assistants. This gives us time to understand the full picture of your health, symptoms, lifestyle, goals, and concerns before you meet with a provider.

We intentionally designed this process to be different from a traditional rushed medical appointment – similar to an expensive concierge clinic, but without the high cost. We achieve this by participating in all major private insurance networks and by providing affordable self-pay alternatives. 


After your intake, you’ll meet with one of our medical providers, who will already have reviewed your information ahead of time. That means your appointment can focus less on repeating your story and more on meaningful conversation, education, and care planning.


A Care Team That Knows You


At Horizen Medical, we believe relationships matter in healthcare. 


Patients are paired with both a dedicated provider and Health Coach rather than being scheduled with whichever clinician happens to be available at the time.


Because of this, your care team gets to know you -- not just your symptoms, but your goals, preferences, lifestyle, stressors, history, and the bigger picture of your health journey. 

It also means you don’t have to keep reintroducing yourself or retelling your story at every visit.


More Than “Just” Perimenopause Care


While perimenopause, menopause, and post-menopause care are at the center of what we do, Horizen Medical is designed to support women through many different aspects of their health journey.


Depending on your needs, care at Horizen Medical may include:

  • Hormone Replacement Therapy (HRT) counseling

  • Weight loss counseling and metabolic health support

  • Birth control consultations

  • Lifestyle and behavior change support through ongoing health coaching

  • In-person Annual Well Visits, including PAP smears

  • Mammograms

  • DEXA scans

  • Ultrasounds

  • …and more!


We understand that women’s health is rarely about just one issue. Symptoms, hormones, metabolism, stress, and lifestyle are all deeply connected, which is why our approach focuses on whole-person care rather than isolated concerns.


In addition to in-person visits – such as physical exams or imaging – many of our services can be completed through telehealth, allowing patients to receive ongoing support and care from a convenient location.


Our goal is to make high-quality women’s healthcare feel more accessible, comprehensive, and realistic for busy women navigating real life.


Care That Continues Beyond the First Visit


During your first year with us, patients can expect to connect with their provider approximately 3-5 times depending on their needs and care plan. These follow-up visits are a necessary part of your care, allowing us to continue monitoring your symptoms, adjusting your treatment plan, reviewing your progress, and providing ongoing support as your needs evolve.


An essential aspect of your continued care includes monthly visits with a Health Coach where you’ll address the best lifestyle and behavior changes to support the work you’re doing with your provider. Health Coaches are there to provide accountability, education, and guidance as you work towards your personal health goals.
